The 2010 Season Begins!

The first official day for FHSAA boys lacrosse is Monday, January 18, 2010. Prior to trying out for the lacrosse team, all players must have a physical and complete all of the forms required by the FHSAA and the Nease Athletic Department. The doctor performing the physical must complete the official FHSAA form for the 2009-2010 school year. All forms can be obtianed from the forms link on this page. Players must turn in all forms to Andrea Johnson in the trainer's office at Nease prior to the start of practice on 1/18/2010. Players who have already submitted their forms for a fall or winter sport do not have to turn in new forms.

The five forms that are required to practice are:

FHSAA Pre-Participation Physical Form

FHSAA Consent and Release Form

Student Athlete Code of conduct

Victory With Honor for Athletes

Victory With Honor for Parents
