REQUIRED FORMS

 

The following forms are required to participate in Boys Lacrosse at Nease H.S. Please download these forms, complete them and bring them to the kickoff team meeting in December. The forms indicated with the ** must be submitted for Lacrosse even thought they may have been submitted for another sport.

1. **PLAYER/PARENT INFO - This form requires a US Lacrosse Membership number and expiration date. All players must be a member of US Lacrosse in order to participate in lacrosse.  Register on-line at www.uslacrosse.org the cost is $35.00 annually.

2. **PARENT CONSENT

3. PHYSICAL - The FHSAA requires medical forms to be filled out by a physician each year.  If you play another sport and already have a medical form on file, please indicate the sport on the form. This physical is valid for 365 days.

8. FIELD TRIP FORMS - The varsity team will likely be making an overnight trip. Since anyone may be selected to play on the varsity (at any time) everyone must complete this form. Please complete Page 4-6.

 

OTHER FORMS

PARENT VOLUNTEER APPLICATION - All adults who volunteer for Nease in any capacity must complete this application each year.  The Boy's Lacrosse Team needs your help in concessions and other activities.  If you would prefer to fill out an on-line application, go to Nease On-line Application and follow the instructions. 

Golden Panther Booster Club Membership - The school district pays for referee fees and partially for field maintenance and transportation.  The Boys Lacrosse Program must pay for uniforms, goals, balls, sandwiches for travel, team dinners, coach buses, as well as many other items. This is done through the generous contributions of our parents as well as from fund-raising activities and concessions proceeds.   By becoming a Golden Panther Booster Club member and designating funds to Boys Lacrosse, you will receive other benefits (parking passes, game passes, apparel discounts, concession coupons, etc.).  The first $75 goes to the general fund that supports capital upgrades, liability insurance, accounting costs and other sports related costs.  The remainder will go directly to Girls Lacrosse. Nease High School requires a 2.0 GPA in order to participate in any Varsity Sport.
